- Use this form for one or two items. Submit an additional form if you have more items.
- Complete ALL data fields about yourself and the item you need. Incomplete information will delay processing of your document delivery request.
- Email notification -- You will be notified by email regarding this document delivery request.
- Online Fulltext -- If an item is found online/fulltext - that item is sent to your email address instead of sending photocopies.
- Print & media items are delivered to the ACC location indicated on the form. If home delivery is requested - be sure to indicate that under "Other Location" and provide mailing address.
- This form is limited to use by currently enrolled ACC distance learning students and faculty (BSP, FBG, RRH, SMC, and distance-online).
