• Use this form for one or two items. Submit an additional form if you have more items.
• Complete ALL data fields about yourself and the item you need. Incomplete information will delay processing of your document delivery request.
• Email notification -- You will be notified by email regarding this document delivery request.
• Online Fulltext -- If an item is found online/fulltext - that item is sent to your email address instead of sending photocopies.
• Print & media items are delivered to the ACC location indicated on the form. If home delivery is requested - be sure to indicate that under "Other Location" and provide mailing address.
• This form is for use by currently enrolled ACC distance learning students (BSP, ECR, FBG, SMC, ECHS, ECS, and distance-online).
• Early College students may also use this form to request document delivery assistance. If you are an ECS or ECHS student - select "OTHER" location, and provide the details about your location.
• Students enrolled at any of the 8 ACC campuses should contact the Reference Desk at their campus library for assistance.
